Clean up codes in InstCombiner::SimplifyDemandedBits():
1. Line out nested call of APInt::zext/trunc.
2. Make more use of APInt::getHighBitsSet/getLowBitsSet.
3. Use APInt[] operator instead of expression like "APIntVal & SignBit".
